• Certified/Validated USYSA Team Roster
• Guest Player Roster approved/validated by the State Association where the player plays
• Copy of Players, Guest Players and Coaches Cards-Both sides of cards
• Concussion Diagnosis Release Team Signature Form
• Dublin Waiver Team Roster Signature Form (Boys Weekend Only)

USYSA and Canadian Teams

• Certified/Validated USYSA Team Roster
• Guest Player Roster approved/validated by the State Association where the player plays
• Copy of Players, Guest Players and Coaches Cards-Both sides of cards
• Permission to travel form if team is outside of your State
• Coach's Concussion Certificate
• Concussion Diagnosis Release Team Signature Form
• Lindsay’s Law Team Roster Signature Form
• Dublin Waiver Team Roster Signature Form (Boys Weekend Only)

US Club Teams

• Certified/Validated USCLUB Team Roster
• Player Loan Form approved by both teams
• Copy of Players, Guest Players and Coaches Cards-Both sides of cards
• Coach's Concussion Certificate
• Concussion Diagnosis Release Team Signature Form
• Lindsay’s Law Team Roster Signature Form
